Initial Consultation:

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Surgery can often be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment at a later date.

Your Visit

Please assist us by providing the following information at the time of your consultation:

- Your surgical referral slip and any x-rays if applicable.

- A list of medications you are presently taking as well as all doctors names and phone numbers.

- Medical or dental Insurance card (if applicable) - any medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, etc.

- ANY X-RAYS that your dentist or physician has taken. Your doctor can email or mail them to our office. If additional films are necessary they can be taken at our office.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

Before Anesthesia:

- A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery.

- Please have nothing to eat or drink eight hours prior to your surgery.

- A responsible adult must accompany the patient to the office, remain in the office during the procedure, and drive the patient home.

Post Surgery:

- You may not operate a motor vehicle 24 hours after surgery

- Dr. Salmon will personally follow up with you the afternoon/evening following your surgery

- You will also be provided with an after hours phone number that you can use to call if you have any questions or concerns.


For your convenience, we accept Visa, MasterCard, American Express, Discover and CareCredit. We deliver the finest care at the most reasonable cost to our patients; therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have insurance, we will obtain an estimate of your benefits prior to scheduled procedures. This may require a consultation with the doctor performing the surgery for a pre-operative evaluation. The estimate is based on the information provided by your insurance and is not a guarantee of benefits, coverage or payment. We collect the estimated patient portion on the date of service. All professional services are filed to your insurance as a courtesy. Any remaining balance after insurance processes will become patient responsibility. The amount your insurance allows for services rendered may be less than our office fees. Deductibles and co-insurance may attribute to receiving a statement after services are rendered and all claims have completed processing by your insurance.
